Diabetic with Nephrotic Syndrome: A Case of “Masked” Membranous Nephropathy

A 63-year-old man with diabetes and hypertension presented with nephrotic syndrome. Biopsy suggested Class 2 diabetic nephropathy, but electron microscopy showed stage 2 membranous nephropathy.
Initial immunofluorescence was negative; pronase digestion revealed masked IgG-κ deposits. He was treated with rituximab and steroids, resulting in reduced proteinuria.
